  • Get your check in the mail
Seems Straight Forward,how many of us don't save receipts for everyday items and lose out on money that we are rightfully entiltled to

It's important to note that filing a claim for something you are not entitled to is illegal.

In most class action settlements, if too many people file claims, no one gets the money. If not enough people request the money, the additional is donated to a charity.

"Under penalty of perjury, all information provided in this Claim Form is true and correct to the best of my knowledge and belief.*

^^ You will always have to sign something similar to the above statement, so just fyi.

going to bookmark this site, i sometimes get class action notices in the mail and often misplace them. this will be helpful.

@chuckydealpl - Same here. I've lost a few.

Or if you pay with cash, they typically will have no way to even notify you that you are part of the settlement
