Says the trial is only valid for the Skype name it was sent to.

The free calls are only to the 36 countries shown on the map, not "anywhere in the world".

One thing to be aware of- you probably have to manually cancel this plan after the first month to not get charged the monthly fee after that. So if you get this plan- put it on your calendars to do so. Can anyone else confirm this?

Yes, you are correct. Here's what it says at the bottom of the page:

"Subscriptions are free for the first month. After this, your subscription will automatically continue and payments will be charged monthly unless you cancel"
